This policy relates to the personal information that you, your Veterinary Practice or Medical professional supplies to DYG Physiotherapy and my use of that data during the administration of treatment and care. This policy was last reviewed and updated in May 2018.

So, I can address you properly and identify you when I make contact or you contact me

So, I can identify you and contact you in relation to your treatment or management of animal treatment

Your Veterinary surgeons / practice / medical professional details

So, I can: –

Veterinary services ensure legal requirement of referral and treatment of care

Medical professionals for multi-disciplinary approach to care if required.

Your animals’ details including their name/s, date of birth/s and working relationship (ie pet /working dog/ agility)

For formal management of the animals’ care and identify them.

 

DYG Physiotherapy may use this data for:

·         Processing of client payments for treatment provided

·         Improvement of my services to you

Who will we share your information with?

·         Your Veterinary Practices as required

·         Medical professionals involved in your care, as required

·         Your insurance company when direct by you

If you would prefer that DYG Physiotherapy did not hold any of the above information then services of care cannot be provided.

How long does DYG Physiotherapy retain your personal data for?

Your personal data will be held with me and processed by me for as long as the purpose for which it was collected continues. Once you have decided that you no longer require our services, your data will be held on our systems for a further seven years. The reasons for this are:

·         To respond to any future queries or complaints.

·         To comply with legal, regulatory and accounting requirements.
